For your convenience, we have outlined our terms, conditions, and policies to help ensure a memorable, relaxing experience.

To respect our clinicians’ professional time and other clients' time, we kindly request at least 24 hours' notice for cancellations or appointment rescheduling. To secure your appointment, we require a credit card at booking. If you cancel with less than 24 hours' notice, we will charge a flat fee of $50. Please note that consultations are complimentary, but we require a credit card to book, and we will charge a $50 fee for cancellations made with less than 24 hours' notice.

Rushing to an appointment doesn't contribute to an enjoyable experience. To make the most of your visit at Suzi Q Wellness / Powered By: Arrive Health & Wellness, we recommend arriving at least 15 minutes early. Our scheduling is thoughtfully designed to ensure you receive the full benefits of your scheduled time with our providers. If you are running late, we will do our best to accommodate you, although we cannot guarantee the full duration of the service.

For Mobile Visits, please be at the designated location at the scheduled time. If you are not present, we will do our best to accommodate you, although we cannot guarantee the service.

Your satisfaction is our priority. We take great pride in the quality of services provided at Suzi Q Wellness / Arrive Health and Wellness. If, for any reason, you are unsatisfied with your service, please inform us within 48 hours, and we will gladly work to ensure your happiness.

For Botox and other neuromodulators, we offer touch-up treatments if you're unsatisfied with the initial results. Please note that we ask you to wait 10 days post-treatment to allow for full effect. However, any requests made more than 20 days after the initial appointment will be outside our touch-up window.

1. Purpose of Evaluation:

The medical evaluation at Suzi Q Wellness /Arrive Health and Wellness is designed to assess your health status and determine the most appropriate treatment plan for your needs. This evaluation involves a comprehensive review of your medical history, current health conditions, and relevant symptoms.

2. Non-Guaranteed Approval:

Undergoing a medical evaluation does not guarantee approval for treatment at Arrive Health and Wellness. Approval for any treatment will be based on the evaluation results and the professional judgment of our healthcare providers.

3. Non-Refundable Fees:

The fees for the medical evaluation are non-refundable. Payment is required at the time of service, and refunds will not be issued regardless of the evaluation outcome or subsequent approval status for treatment.

4. Confidentiality

All information disclosed during the evaluation will be kept confidential in accordance with our privacy policy and applicable laws. We will use your health information solely to determine your treatment plan.

5.Compliance

By participating in the medical evaluation, you agree to provide accurate and complete information about your health history and current condition. Failure to do so may affect the accuracy of the evaluation and the suitability of the recommended treatment plan.

6. Modifications to Terms

Suzi Q Wellness / Arrive Health and Wellness reserves the right to modify these terms and conditions at any time. Any changes will be communicated to you before your evaluation. By proceeding with the medical evaluation, you acknowledge that you have read, understood, and agree to these terms and conditions.

We accept all major credit cards, including HSA and FSA. Please be aware that all prices are subject to change without prior notice.
